Passport Requirements

First Time Applications

  • Original Panamanian Birth Certificate (It must have the authentication seals)
  • 2 sets of the Application Form (signed by both parents)
  • 2 sets of Parents Authorization Form, (signed by both parents). “First Time  application”
  • 2 sets of the Signature Certification Form –For  applicants over 13 years old
  • 2 sets of the Signature Form – For  applicants over 13 years old
  • 2 copies of Father’s and Mother’s Panamanian Cedula or passport– Present Original for validation.
  • Four (4) recent pictures: color passport size photos /focus facing front/ 35mm X 45mm / white background; (use a proper attire; no sleeveless and cleavages); no piercing, no big earrings and no contact lenses or glasses), no hair clips nor ribbons, neck and shoulders needs to be shown.
  • If the applicant is underage (below 18 years old) both parents must sign the application form and the authorization letter.

NOTE: For single or divorce parents they must present the custody and/or divorce documents (if applicable) or made a notarized declaration of their status. (Click here  to download the declaration’s template).

Passport Renewal

  • 2 sets of the Application Form signed and completed
  • 2 sets of the Passport Renewal Form 
  • 2 sets of the Renewal Form for Passports out of pages 
  • 2 sets of the Signature Form – For applicants over 13 years old
  • 2 sets of the Signature Certification Form –For applicants over 13 years old 
  • 2 copies of the Panamanian Birth Certificate (It must have the authentication seals)
  • 2 copies of the current passport (only first and second page) — Present Original for validation.
  • 2 copies of your Panamanian Cedula — Present Original for validation.

Lost or Stolen passports

  • 2 sets of the Application Form signed and completed
  • 2 sets of Signature Form (for all applicants over 13 years old)
  • 2 sets of the Signature Certification Form – Only for applicants over 13 years old.
  • 2 sets of Passport Renewal Form for Lost or Stolen Passport
  • 2 sets of Personal Lost or Stolen Passport Report
  • Original Report of Lost/Stolen Passport issued by the local Police authorities
  • Spanish Translation of the Official  Police Report
  • Copy of first and second page of the lost passport (if available)
  • Copy of the Panamanian Cedula (if available)
  • Four (4) recent pictures: color passport size photos /focus facing front/ 35mm X 45mm / white background; (use a proper attire; no sleeveless and cleavages); no piercing, no big earrings and no contact lenses or glasses), no hair clips nor ribbons, neck and shoulders needs to be shown.

Passport renewals for Married and Divorced Persons 

Requirements to be submitted :

  • 2 sets of duly accomplished Application Form
  • 2 sets of Signature Form A
  • Letter in Spanish requesting the cancellation of the current passport and for the issuance of a new one. (download here the template)
  • Four (4) recent pictures: color passport size photos /focus facing front/ 35mm X 45mm / white background; (use a proper attire; no sleeveless and cleavages); no piercing, no big earrings and no contact lenses or glasses), no hair clips nor ribbons, neck and shoulders needs to be shown.

Original and 2 copies of the following documents:

  • Passport  ( first and second page)
  • Cedula
  • Birth Certificate

NOTE:

Applicants who have changed their single status to married should register their Marriage at the Panama’s Civil Registry Office and request a new Cedula before requesting the change in their passport.

Applicants who got divorce must first register their divorced in the Civil Registry Office and request a new Cedula before requesting the change of their passport.

Applicants who are married and have adopted their spouse’s last name, but wish to continue using their maiden name, should make the appropriate application at the Civil Registry and request a new Cedula.
