Application for a Duplicate
Requirements for obtaining vital records (duplicates) from Panama Civil Registry Office.
For Applications of Duplicates of: Birth Certificates, Marriage Certificates and Death Certificates.
The applicant must submit the following documents:
- A signed letter making the request is needed for obtaining the duplicate of vital records
- Provide the following information as it might be need for each case.
Birth Certificates:
- Full name of the applicant
- Place (city / county) of birth
- Date of birth
- No. Insc / cedula
- Father’s full name
- Mother’s full name
- Fees as established by the Civil Registry
Marriage Certificates:
- Full names of the applicants, the bride and groom
- No. Insc / cedula
- Copy of the Cedula identification number of bride and groom
- Date of marriage
- Place (city / county) of marriage
- Both parents full names
- Fees as established by the Civil Registry
Death Certificates:
- Full name of the deceased
- No. Inscription/cedula
- Date of death
- Place (city /county) of death
- Fathers full name
- Mothers full name
- Fees as established by the Civil Registry
*In some case some more documents may be requested by the Panamanian Civil Registry Office.
